Colorectal cancer Colorectal cancer is the third most common malignant tumor worldwide. Globally, more than 600.000 people die from colorectal cancer every year, mainly from metastatic disease. Before the era of chemotherapy, patients with mCRC had a median overall survival of 9 months. With the introduction chemotherapeutic agents and targeted agents against VEGF and the EGF receptor, overall survival has improved to over 30 months. Originally, when patients presented with metastases, the disease was viewed as always being widespread and patients have typically been treated with systemic therapy. However, local treatment of metastases is technically feasible in an increasing number of patients with mCRC by either resection, radiofrequency ablation or stereotactic radiotherapy. Preliminary and retrospective evidence suggests that patients with extensive multi-organ mCRC may benefit from local treatment of metastases. This results in a growing debate in multidisciplinary teams on whether local treatment of metastases should be performed in these patients. Part One: Debulking in metastatic colorectal cancer In Chapter 2, the rationale of debulking therapy for patients with multi-organ mCRC is reviewed. The ORCHESTRA trial is the first multicenter randomized clinical trial in which is evaluated if multidisciplinary local treatment in addition to first line palliative systemic treatment (NCT01792934) results in survival benefit for patients with extensive mCRC. A preplanned safety and feasibility evaluation was performed aftercompletion of study treatment of the first 100 patients enrolled in the trial and reported in Chapter 3. A translational study program was embedded in the ORCHESTRA trial protocol and blood and tissue samples were collected from all patients enrolled.

In this book, a distinguished group of clinicians provide straightforward, focused answers to the questions most commonly confronted by a multidisciplinary team when caring for patients with rectal cancer. The format of the book is designed to aid optimal decision making in a multidisciplinary setting with respect to key issues in imaging, radiotherapy, chemotherapy, and surgery, all of which are considered in detail. Helpful guidance is also offered on risk factor identification, pathology procedures and their prognostic value, and multidisciplinary team management. The authors have been carefully selected for their expertise, and most have been involved in the management of the more significant trials published in Europe during the past decade. In an era in which all patients are entitled to access healthcare systems that deliver the highest quality of treatment within a safe environment, together with appropriate advice, support, and long-term follow-up, the multidisciplinary team is of central importance. This successful book, in its revised and updated second edition, is designed to provide all the guidance likely to be required by members of the multidisciplinary team at various stages of the pathway of care for patients with rectal cancer.

This text provides information currently not available from any single source. It summarizes state-of-the-art issues related to the treatment of colorectal cancer including descriptions of optimal multidisciplinary care utilizing all available modalities as well as related issues such as screening and prevention, management for special populations, psychological support and emerging directions in future treatment. Over the course of the last decade, the treatment of colorectal cancer has evolved quite rapidly. New scientific and clinical advances have modified the standard of care and led to improved patient outcomes. At the same time, the treatment of colorectal cancer has become increasingly complex, requiring the comprehensive review and assessment of multiple issues including genetics, radiology, surgery, molecular diagnostics, chemotherapy, and more. As a result the harmony and open communication between these specialties facilitated by a multidisciplinary team approach are crucial in providing the best care to patients and ensuring successful treatment.

Rectal cancer is one of the most prevalent cancers world-wide. It is also a paradigm for multimodal management, as the combination of surgery, chemotherapy and radiotherapy is often necessary to achieve the optimal outcome. Recently, international experts met in Heidelberg, Germany to discuss the latest developments in the management of rectal cancer, including the anatomic and pathologic basis, staging tools, surgical concepts including fast-track surgery and laparoscopic resection, functional outcome after surgery and the role of radio- and chemotherapy. This monograph summarizes this meeting and gives an extensive overview of the current concepts in management of rectal cancer.
