The book is the first of its kind to provide a comparative analysis of the provision of social and public services in France, Italy, Germany, the UK and Norway. What are public health services? Countries across Europe understand what they are or what they should include differently. This study describes the experiences of nine countries detailing the ways they have opted to organize and finance public health services and train and employ their public health workforce. It covers England France Germany Italy the Netherlands Slovenia Sweden Poland and the Republic of Moldova and aims to give insights into current practice that will support decision-makers in their efforts to strengthen public health capacities and services. Each country chapter captures the historical background of public health services and the context in which they operate; sets out the main organizational structures; assesses the sources of public health financing and how it is allocated; explains the training and employment of the public health workforce; and analyses existing frameworks for quality and performance assessment. The study reveals a wide range of experience and variation across Europe and clearly illustrates two fundamentally different approaches to public health services: integration with curative health services (as in Slovenia or Sweden) or organization and provision through a separate parallel structure (Republic of Moldova). The case studies explore the context that explain this divergence and its implications. But often lost in the heat of debate is the fact that vouchers are just another tool in the government's tool chest, a restricted subsidy that falls somewhere between the extremes of cash and direct government provision of services. The instrument itself is not new—the 1944 GI Bill of Rights was a voucher, and vouchers for food, college aid, and housing have been in place for decades. Until now, however, the study of vouchers has been restricted to a few controversial applications. This volume, which grew out of a conference sponsored by the Brookings Institution, the Urban Institute, and the Committee for Economic Development, fills the gap, offering a framework for comparative analysis of specific policy issues related to vouchers. Its 16 essays address the economics, politics, and legal issues of voucher use and explore how vouchers are currently employed in the United States and abroad for education, child care, job training, housing, and health care. C. Eugene Steuerle is a senior fellow at the Urban Institute and has worked under four different U.S. presidents on a variety of reform issues in such areas as social security, budget, tax, and health policy. Robert D. Reischauer, a senior fellow in Economic Studies at the Brookings Institution, was director of the Congressional Budget Office from 1989 to 1995. George Peterson is a senior fellow at the Urban Institute; from 1976 to 1985 he directed the Institute's Public Finance Research Center. Van Doorn Ooms, senior vice president and director of research at the Committee for Economic Development, was formerly executive director for policy and chief economist of the Committee on the Budget, U.S. House of Representatives, 1989-1990, and was the Budget Committee's chief economist from 1981 to 1988.
This open access book presents a topical, comprehensive and differentiated analysis of Germany’s public administration and reforms. It provides an overview on key elements of German public administration at the federal, Länder and local levels of government as well as on current reform activities of the public sector. It examines the key institutional features of German public administration; the changing relationships between public administration, society and the private sector; the administrative reforms at different levels of the federal system and numerous sectors; and new challenges and modernization approaches like digitalization, Open Government and Better Regulation. Each chapter offers a combination of descriptive information and problem-oriented analysis, presenting key topical issues in Germany which are relevant to an international readership.
How are public health services in Europe organized and financed? With European health systems facing a plethora of challenges that can be addressed through public health interventions there is renewed interest in strengthening public health services. Yet there are enormous gaps in our knowledge. How many people work in public health? How much money is spent on public health? What does it actually achieve? None of these questions can be answered easily. This volume brings together current knowledge on the organization and financing of public health services in Europe. It is based on country reports on the organization and financing of public health services in nine European countries and an in-depth analysis of the involvement of public health services in addressing three contemporary public health challenges (alcohol obesity and antimicrobial resistance). The focus is on four core dimensions of public health services: organization financing the public health workforce and quality assurance. The questions the volume seeks to answer are: o How are public health services in Europe organized? Are there good practices that can be emulated? What policy options are available? o How much is spent on public health services? Where do resources come from? This open access State-of-the-Art Survey describes and documents the developments and results of the Once-Only Principle Project (TOOP). The Once-Only Principle (OOP) is part of the seven underlying principles of the eGovernment Action Plan 2016-2020. It aims to make the government more effective and to reduce administrative burdens by asking citizens and companies to provide certain standard information to the public authorities only once. The project was horizontal and policy-driven with the aim of showing that the implementation of OOP in a cross-border and cross-sector setting is feasible. The book summarizes the results of the project from policy, organizational, architectural, and technical points of view.
Swiss citizens approve of their government and the way democracy is practiced; they trust the authorities and are satisfied with the range of services Swiss governments provide. This is quite unusual when compared to other countries. This open access book provides insight into the organization and the functioning of the Swiss state. It claims that, beyond politics, institutions and public administration, there are other factors which make a country successful. The authors argue that Switzerland is an interesting case, from a theoretical, scientific and a more practice-oriented perspective. While confronted with the same challenges as other countries, Switzerland offers different solutions, some of which work astonishingly well.
Future growth in the countries in Eastern Europe and Central Asia (ECA) will increasingly depend on innovation. And innovation requires skills. This makes it important, as countries plan for recovery, to undertake reforms to reduce the skills shortages that the previous growth episode exposed. Education systems have a very important role to play in creating the right skills. But education systems in the region fall short of the demands of their economies in two major ways. The first is that despite high levels of enrollment they do not produce enough graduates with the right skills. Students graduate with diplomas, not with skills, because the quality of the education for many students is poor. In large part this is because education systems remain focused on providing an excellent education to a few at the expense of improving the quality of learning for the majority. Moreover, the systems are still making the transition from teaching the basics to inculcating higher order skills such as critical-thinking and problem solving. The second way in which education systems fall short is that outside of a few countries in the EU there are few opportunities for adults to retrain, or acquire new skills. This book argues that generating more of the right skills requires a fundamental change of approach in the education systems in the region so that they aim for, and deliver, higher quality education for the vast majority of students (not just diplomas but skills). To start with, education systems need to turn the lights on and take seriously the measurement of what students actually learn as opposed to measurement of the inputs into the education process on the implicit assumption that learning follows. Policy makers also need to move away from the focus on inputs and processes and increase the emphasis on incentives.
