Introduction : L'IVG est un acte douloureux dont la prise en charge ne fait pas l'objet de recommandations. Objectif : Identifier les moyens médicamenteux et non médicamenteux de prise en charge la douleur lors de l'IVG médicamenteuse. Matériel et méthode : Revue systématique de la littérature d'études réalisées entre 1989 et 2016 concernant la prise en charge de la douleur lors d'une IVG médicamenteuse. Résultats : Huit articles ont été analysés. L'ibuprofène a montré la plus grande efficacité en comparaison au placebo et au paracétamol. Son administration de manière prophylactique n'a pas montré d'intérêt en matière de réduction de la douleur. La prise de paracétamol-codéiné diminue la consommation d'antalgiques supplémentaires. Des méthodes non médicamenteuses ont été validées (éléctroacupuncture et dilatateur cervical). Cependant les protocoles d'IVG ne sont pas standardisés et ont rendu difficile la comparaison des résultats. Des facteurs prédictifs de douleur plus intense ont été mis en évidence tels que la dysménorrhée, les faibles parité et gestité, et l'âge gestationnel avancé. Conclusion : Il existe des thérapeutiques efficaces dans la prise en charge de la douleur. Cependant, il apparaît nécessaire de poursuivre la recherche dans ce domaine.
Introduction : L’interruption volontaire de grossesse (IVG) par méthode médicamenteuse est un soin douloureux. OBJECTIF : Evaluation de la douleur ressentie (moment et intensité) par les femmes et recherche de facteurs prédictifs de survenue de douleur intense au cours d’une IVG médicamenteuse. Matériel et méthode : Etude observationnelle, prospective, réalisée au Centre d’IVG de l’hôpital Louis Mourier (Colombes, 92) d’avril à septembre 2011, auprès des femmes ayant recours à une IVG médicamenteuse en hospitalisation de journée. Les femmes ayant présenté des complications (rétention, hémorragie) ont été exclues. Le protocole médicamenteux utilisé est : 200mg de mifépristone, suivi à 36 à 48h plus tard de 400g de misoprostol par voie jugale et d’une prémédication antalgique systématique (400mg d’ibuprofène), puis à nouveau 400g de misoprostol 2h plus tard. Les femmes sont hospitalisées une demi-journée le jour de la prise du misoprostol dans un service dédié et sont revue 15 jours plus tard en consultation de contrôle. La douleur ressentie par les patientes est évaluée par l’Echelle Visuelle Analogique (EVA) aux différents moments de l’IVG : après la prise de mifépristone, 2h puis 4h après la première prise de misoprostol puis dans les 4 jours suivant. Les facteurs prédictifs (caractéristiques socio-cliniques) de survenue d’une douleur intense (EVA 5) sont recherchés par régression logistique uni puis multivariée. Résultats : L’étude porte sur 92 patientes. La douleur ressentie au cours de l’IVG médicamenteuse est en moyenne faible à modérée (EVA à 2,7). La douleur atteint une EVA 5 chez 19,5% des femmes après la mifépristone, de 31,5% 2h après le misoprostol, 18,1% 4h après le misoprostol et 36,6% dans les 4 jours suivant l’IVG (différences non significatives). Les facteurs prédictifs de survenue d’une EVA 5 pendant l’IVG sont le terme (OR=4,78 par SA supplémentaire) et le fait d’être accompagnée au CIVG par un proche (OR=4,58). Les facteurs prédictifs de survenue d’une EVA 5 dans les 2h suivant la prise de misoprostol sont la nulliparité (OR=37,92), la nulligestité (OR=3,05), le fait d’être accompagnée au CIVG (OR=7,20), l’intensité des dysménorrhées (OR= 1,40) et l’intensité des douleurs suite à la prise de mifépristone (OR=1,97). Conclusion : Dans cette étude, la douleur moyenne au cours de l’IVG médicamenteuse est faible. Nous n’avons pas mis en évidence de moment plus douloureux au cours de la procédure. En plus de facteurs prédictifs déjà décrits dans la littérature (terme avancé, nulliparité et intensité des dysménorrhées), nous identifions une possible influence de la présence d’un accompagnant. La recherche systématique de l’ensemble de ces facteurs pourrait permettre d’améliorer la prise en charge antalgique.
This book provides an in-depth analysis of the influence of public policy on sex selection. Three Asian countries were chosen for the comparative policy analysis, namely South Korea, India and Vietnam that share in common a historical legacy of son preference, high levels of sex imbalances and active policy response to curbing the growing demographic masculinization of their nations. The research based on the data collected from field work in the three countries shows that despite the adoption of very similar anti-sex selection policies the outcomes have been markedly different for each of the three countries. These unexpected diverse outcomes are explained partly by their different historical and cultural contexts, and partly to the different social, political and economic institutions and dynamics. This monograph offers careful and detailed explanations of both within and across country diversities in policy outcomes, pointing to the importance and the limits of cross-national policy learning and adoption, and raising questions about the efficacy of international organizations’ current approaches to global policy and knowledge transfer.
A decade in preparation, Immigrant Entrepreneurs offers the most comprehensive case study ever completed of the causes and consequences of immigrant business ownership. Koreans are the most entrepreneurial of America's new immigrants. By the mid-1970s Americans had already become aware that Korean immigrants were opening, buying, and operating numerous business enterprises in major cities. When Koreans flourished in small business, Americans wanted to know how immigrants could find lucrative business opportunities where native-born Americans could not. Somewhat later, when Korean-black conflicts surfaced in a number of cities, Americans also began to fear the implications for intergroup relations of immigrant entrepreneurs who start in the middle rather than at the bottom of the social and economic hierarchy. Nowhere was immigrant enterprise more obvious or impressive than in Los Angeles, the world's largest Korean settlement outside of Korea and America's premier city of small business. Analyzing both the short-run and the long-run causes of Korean entrepreneurship, the authors explain why the Koreans could find, acquire, and operate small business firms more easily than could native-born residents. They also provide a context for distinguishing clashes of culture and clashes of interest which cause black-Korean tensions in cities, and for framing effective policies to minimize the tensions.
