However, the assessment of the prescription patterns indicates the lack of evidence-based guidelines for prescribing the drug. The guidelines and recommendations provided in this book may help to clarify the uncertainties related to MTX prescription. The DMARDs discussed in the present volume are second-line drugs used in the management of RA, though they had occupied the first position during the early years of DMARD development. Since the clinical experiences with the drugs like AZA, cyclophosphamide, gold salts, and cyclosporine were good; they were the first treatment of choice during the 70s to 90s. Experience with newer group of DMARDs especially MTX and biologics, have altered the treatment recommendations. However, the clinical experience and evidence regarding the use of these drugs are comparatively lesser. A clear consensus and further exploration of their clinical use may help in re-establishing and defining the role. However, The clinical data suggests that these drugs possess marginal toxicity/therapeutic ratio. the purpose of this book is to introduce these drugs to the readers and facilitate the experts to handle these drugs with extreme caution, since they are often used in RA patients who have extra-articular features or those who are refractory to regularly used DMARDs.
