Chapter I introduces the overview of hospital discharge. Chapter II discusses the Transitions’ Theory based on Meleis Middle Range Theory that can be used as the basic framework to guide the discharge transition concept which is relevant to the main concept of hospital discharge. Chapter III illustrates Self-Care Nursing Theory by Dorothea Elizabeth Orem and it’s an application in surgical care. Chapter IV has shown how the discharge planning process is going. Chapter V and VI discuss the discharge teaching process and how to develop a mini-teaching plan using METHOD. Chapter VII describes the discharge readiness for hospital discharge. The Roundtable on Health Literacy brings together leaders from academia, industry, government, foundations, and associations and representatives of patient and consumer interests who work to improve health literacy. To achieve its mission, the roundtable discusses challenges facing health literacy practice and research and identifies approaches to promote health literacy through mechanisms and partnerships in both the public and private sectors. To explore the aspects of health literacy that impact the ability of patients to understand and follow discharge instructions and to learn from examples of how discharge instructions can be written to improve patient understanding of-and hence compliance with-discharge instructions, the Roundtable on Health Literacy held a public workshop. The workshop featured presentations and discussions that examined the implications of health literacy for discharge instructions for both ambulatory and inpatient facilities. Facilitating Patient Understanding of Discharge Instructions summarizes the presentations and discussions of the workshop. This report gives an overview of the impact of discharge instructions on outcomes, and discusses the specifics of inpatient discharge summaries and outpatient after-visit summaries. The report also contains case studies illustrating different approaches to improving discharge instructions.

This open access book aims to provide a comprehensive but practical overview of the knowledge required for the assessment and management of the older adult with or at risk of fragility fracture. It considers this from the perspectives of all of the settings in which this group of patients receive nursing care. Globally, a fragility fracture is estimated to occur every 3 seconds. This amounts to 25 000 fractures per day or 9 million per year. The financial costs are reported to be: 32 billion EUR per year in Europe and 20 billon USD in the United States. As the population of China ages, the cost of hip fracture care there is likely to reach 1.25 billion USD by 2020 and 265 billion by 2050 (International Osteoporosis Foundation 2016). Consequently, the need for nursing for patients with fragility fracture across the world is immense. Fragility fracture is one of the foremost challenges for health care providers, and the impact of each one of those expected 9 million hip fractures is significant pain, disability, reduced quality of life, loss of independence and decreased life expectancy. There is a need for coordinated, multi-disciplinary models of care for secondary fracture prevention based on the increasing evidence that such models make a difference. There is also a need to promote and facilitate high quality, evidence-based effective care to those who suffer a fragility fracture with a focus on the best outcomes for recovery, rehabilitation and secondary prevention of further fracture. The care community has to understand better the experience of fragility fracture from the perspective of the patient so that direct improvements in care can be based on the perspectives of the users. This book supports these needs by providing a comprehensive approach to nursing practice in fragility fracture care.

Before the Scalpel: What Everyone Should Know about Anesthesia demystifies the issues surrounding the types and uses of anesthesia and addresses common fears about wakefulness and pain. Straightforward language, real-life examples, photographs and illustrations guide you. Individual chapters focus on how anesthesia is used in plastic and cosmetic surgery, labor, and for dental care. Other chapters explore the special needs of babies and children, how surgeons and anesthesiologists have adapted their techniques in response to growing obesity rates, and discuss how to avoid post-operative nausea and vomiting. Throughout the book, Dr. Dhar carefully flags issues directly affecting your medical care. Each chapter ends with an invaluable checklist, called prescriptives, that highlights key points and provides space to take notes when you talk with your surgeon or anesthesia provider. The checklists help you ask questions relevant to your case and ensure that you receive and remember the answers.

The purpose of this research study was to investigate the difference between the adult medical-surgical patients' perception of readiness for discharge to home and 30 day readmissions to acute care. A convenience sample of 33 adult patients, aged 18 and greater, being discharged to home were included as participants in this study. The study was conducted on two medical-surgical nursing units at a teaching hospital located in an urban area in the upstate of South Carolina. On the participant's day of discharge, the participant's primary nurse provided the participant with the appropriate discharge instructions as outlined by the healthcare provider. The primary nurse notified the principal investigator the patient was ready for discharge. The principal investigator then approached the participant and explained the study to them including informed consent. If the participant agreed to participate in the research study, he/she was asked to complete a demographic tool and the Readiness for Hospital Discharge Scale (RHDS). An independent samples t test was conducted to determine the difference in how prepared the participant was for discharge based on reason for admission, medical or surgical, using the total test scores and subscale scores on the RHDS. No significant difference was found between the total RHDS score and the reason for admission, t(31) =.383, p >.05. There were no differences in how well prepared medical patients were for discharge (M = 164.95, SD = 37.56) when compared to how well surgical patients were prepared (M = 160.30, SD = 27.55). There were no significant difference in the subscale scores of Knowledge t(31) =.-012, p >.05, Personal Status t(31) =.357, p >.05, Coping Ability t(31) =.020, p >.05, or Expected Support t(31) =.899, p >.05 between the medical and surgical patients. Due to the low number of readmissions, no difference was found between the patients' perception of discharge to home and 30 day readmission rates.

Normal Accidents analyzes the social side of technological risk. Charles Perrow argues that the conventional engineering approach to ensuring safety--building in more warnings and safeguards--fails because systems complexity makes failures inevitable. He asserts that typical precautions, by adding to complexity, may help create new categories of accidents. (At Chernobyl, tests of a new safety system helped produce the meltdown and subsequent fire.) By recognizing two dimensions of risk--complex versus linear interactions, and tight versus loose coupling--this book provides a powerful framework for analyzing risks and the organizations that insist we run them. The first edition fulfilled one reviewer's prediction that it "may mark the beginning of accident research." In the new afterword to this edition Perrow reviews the extensive work on the major accidents of the last fifteen years, including Bhopal, Chernobyl, and the Challenger disaster. The new postscript probes what the author considers to be the "quintessential 'Normal Accident'" of our time: the Y2K computer problem.
