The purpose of this Report is to provide guidance for investigating radiological incidents that can occur wherever radioactive materials are handled, stored, used or transported, or where radiation generating equipment is operated. Radiological incidents have the potential to adversely impact; the health and safety of workers or members of the public, the environment, operations, and compliance with regulations. This Report provides guidance and practical information for individuals who have the responsibility of performing or overseeing investigations to include a scaled approach such that the extent and rigor of the investigation can be tailored to the severity and complexity of the incident. Guidance is provided on appointing individuals to an incident investigation team including recommendations for the training and qualifications of investigators and the use of consultants and specialists in conducting the investigation. The process of investigation includes a discussion of the initial response to the incident, including the procedures for controlling the incident scene to prevent loss of information, recovering any physical items that may have been removed, and how to gather information related to the incident. Various aspects for conducting the investigation are discussed including the initial team meeting, performance of onsite inspections, interviewing personnel involved in the incident, and collecting physical evidence. Performance of the cause analysis is reviewed including which type of cause analysis to perform. Ideas for the development of a corrective action plan and preparation of the investigation report, including legal considerations, are provided along with suggestions for scheduling, reviewing, tracking and trending the effectiveness of corrective actions. Accidents and terrorist attacks that lead to the release of radioactive materials can cause deaths, injuries, and a range of psychosocial effects in the surrounding community and team of emergency responders. In the United States, federal, state, and local agencies respond with the necessary resources to address the consequences of nuclear and radiological incidents and monitor the affected population. Following the 2011 Fukushima Daiichi Nuclear Power Plant accident and the 2017 Gotham Shield National Level Exercise, the CDC recognized an opportunity to improve their practices by establishing a more efficient and timely health effect surveillance system before another incident occurs. On March 12-13th, 2019, the National Academies convened a workshop to discuss the process for preparing a radiation registry for monitoring long-term health effects of populations affected by a nuclear or radiological incident. Participants assessed existing information, useful practices, and tools for planning a radiation registry that will enhance incident monitoring and response methods. This publication summarizes the discussions and presentations from the workshop.

Under the Convention on Assistance in Case of a Nuclear Accident or Radiological Emergency, the Georgian authorities requested assistance from the IAEA in relation to the 2001 radiological accident in Lia. This assistance related to advice on the dose assessment, source recovery and medical management of those involved in the accident. This report provides the detailed information on the accident and presents the findings and conclusions and lessons learned from the treatment of the overexposed victims. The aim is to help to avoid similar occurrences by improving safety, and to minimize the consequences of any such events that do occur.

The Science of Responding to a Nuclear Reactor Accident summarizes the presentations and discussions of the May 2014 Gilbert W. Beebe Symposium titled "The Science and Response to a Nuclear Reactor Accident". The symposium, dedicated in honor of the distinguished National Cancer Institute radiation epidemiologist who died in 2003, was co-hosted by the Nuclear and Radiation Studies Board of the National Academy of Sciences and the National Cancer Institute. The symposium topic was prompted by the March 2011 accident at the Fukushima Daiichi nuclear power plant that was initiated by the 9.0-magnitude earthquake and tsunami off the northeast coast of Japan. This was the fourth major nuclear accident that has occurred since the beginning of the nuclear age some 60 years ago. The 1957 Windscale accident in the United Kingdom caused by a fire in the reactor, the 1979 Three Mile Island accident in the United States caused by mechanical and human errors, and the 1986 Chernobyl accident in the former Soviet Union caused by a series of human errors during the conduct of a reactor experiment are the other three major accidents. The rarity of nuclear accidents and the limited amount of existing experiences that have been assembled over the decades heightens the importance of learning from the past. This year's symposium promoted discussions among federal, state, academic, research institute, and news media representatives on current scientific knowledge and response plans for nuclear reactor accidents. The Beebe symposium explored how experiences from past nuclear plant accidents can be used to mitigate the consequences of future accidents, if they occur. On 21 June 1990 a fatal radiological accident occurred at an industrial irradiation facility at Soreq, Israel. An operator entered the irradiation room by circumventing safety systems and was acutely exposed, with an estimated whole body dose of 10-20 Gy. The accident, like earlier accidents at similar irradiators, was the consequence of the contravention of operating procedures. An IAEA review team investigated the causes of the accident. This report presents its findings and recommendations and describes the clinical management of the patient, particularly of the haematological phase. The medical treatment included the use of emerging therapies with haematopoietic growth factor drugs which may rescue the overexposed patient, albeit in this case only temporarily. The report is intended for regulatory authorities responsible for the regulation and inspection of irradiators, operating organizations and physicians who may need to treat overexposed patients.

The radiological accident described in this report took place in Lilo, Georgia, when sealed radiation sources were abandoned by a previous owner at a site without following established regulatory safety procedures. As a consequence, 11 individuals at the site were exposed for a long period of time to high doses of radiation which resulted inter alia in severe radiation induced skin injuries. The present report, which is co-sponsored by the World Health Organization, provides information on the medical management of radiation induced skin injuries as well as a comprehensive report on the circumstances and details of the accident and the lessons to be learned.
