A richly illustrated guide to general surgical procedures suitable for use in small hospitals that are subject to constraints on personnel, equipment, and drugs. Information is addressed to the medical officer who may not have formal training in surgery, yet has gained experience under supervision of all the relevant techniques. Following an overview of basic principles, the book presents detailed information on surgical procedures for the face and neck, chest, abdomen, gastrointestinal tract, and urogenital system. Pediatric surgery is also covered. Most of the operations included are for saving life, alleviating pain, preventing the development of serious complications, or stabilizing a patient's condition pending referral. Simple but standard surgical techniques have been selected wherever possible, and procedures that require specialist skills or that could add unnecessarily to the doctor's workload have been avoided. Lists of essential surgical instruments, equipment and supplies conclude the text.

Many patients who present to district (first-referral) level hospitals require surgical treatment for trauma, obstetric, abdominal or orthopedic emergencies. Often surgery cannot be safely postponed to allow their transfer to a secondary or tertiary-level hospital but many district hospitals in developing countries have no specialist surgical teams and are staffed by medical, nursing, and paramedical personnel who perform a wide range of surgical procedures often with inadequate training. The quality of surgical and acute care is often further constrained by poor facilities, inadequate low-technology apparatus and limited supplies of drugs, materials, and other essentials. The mission of the team responsible for Clinical Procedures in the World Health Organization Department of Essential Health Technologies (EHT) is to promote the quality of clinical care through the identification, promotion and standardization of appropriate procedures, equipment and materials, particularly at district hospital level. WHO/BCT has identified education and training as a particular priority, especially for non-specialist practitioners who practice surgery and anesthesia. It has therefore developed Surgical Care at the District Hospital as a practical resource for individual practitioners and for use in undergraduate and postgraduate programs in-service training and continuing medical education programs. The manual is a successor of three earlier publications that are widely used throughout the world and that remain important reference texts: General Surgery at the District Hospital (WHO 1988), Surgery at the District Hospital: Obstetrics Gynecology Orthopedics and Traumatology (WHO 1991), Anesthesia at the District Hospital (WHO 1988; second edition 2000). This new manual draws together material from these three publications into a single volume which includes new and updated material, as well as material from Managing Complications in Pregnancy and Childbirth: A Guide for Midwives and Doctors (WHO 2000).

An illustrated guide to essential surgical procedures for treating the major complications of pregnancy and childbirth and for managing traumatic injuries, including fractures and burns. Gynecological procedures commonly required in small hospitals are also covered. Throughout the book, emphasis is placed on standard surgical protocols that represent the safest line of action in hospital settings where equipment may be primitive, drugs limited, and specialist services sparse. The first six chapters describe essential procedures for treating the major complications of pregnancy and childbirth and for preventing maternal deaths. Life-threatening emergencies, such as obstructed labor and ruptured ectopic pregnancy, are covered together with more general obstetric procedures. Gynecological operations form the focus of four chapters, which describe general gynecological procedures, techniques for female sterilization and IUD insertion, and procedures for obtaining biopsy or cytological samples from the endometrium, cervix, and vulva. The remaining 14 chapters describe essential techniques in orthopedics and traumatology, providing details on both basic orthopedic techniques and the management of specific fractures, dislocations, burns, and other injuries.
