Pain management is a growing field of medical practice in the United States. In pain management practices, urine drug testing has emerged as the preferred method for medication compliance monitoring. A pain management clinic built its own drug testing laboratory, using a liquid chromatography tandem mass spectrometry testing method developed by a reference laboratory. Before accepting patient samples, the pain management laboratory validated the method to confirm its reliability. Validation included confirmation of, among other things, the accuracy, precision, and trueness of the method, the carryover on the instrument, and the stability of the samples. Once validated, the beta-glucuronidase hydrolysis described in the method was optimized for morphine conversion from morphine-3-glucuronide. Optimization experiments focused on the incubation time, incubation temperature, enzyme starting concentration and hydrolysis solution pH used for the hydrolysis reaction described in the method. Using analysis of variance and Dunnett's multiple comparisons test, the hydrolysis reaction was confirmed to produce optimum morphine conversion.
