Abstract: Objective: The aim of the current study was to investigate the differences in baseline characteristics and three-year outcomes between two diagnostic categories with presentation of first-episode psychosis: bipolar affective disorder (mania with psychotic features) and schizophrenia. The comparison was based on pre-treatment characteristics, clinical presentation, symptomatic and functional outcomes, and engagement in risk behaviours. Methods:461 schizophrenic patients and 54 bipolar affective disorder (BAD) patients between the ages of 15 to 25 years from a local first-episode psychosis treatment program within the years2001 to 2003 were studied. Researchers collected detailed data on baseline and three-year follow up variables from systematic medical file review for statistical analyses. Results: At service entry, compared to schizophrenic patients, bipolar affective disorder(BAD)patients exhibited more prominent positive symptoms (p = 0.01), were younger at first presentation and had a higher unemployment rate (p Conclusion: The outstanding baseline poorer functioning level of bipolar affective disorder patients have progressively made a modest improvement in functional outcomes at the end of three-year follow up. BAD patients also displayed a marked improvement with fewer positive symptoms in the follow up. The results suggest a differentiation in symptomatology and the course of illness between bipolar affective disorder and schizophrenia with first-episode psychosis. Several contributions in our first book about schizo affective disorders (Marneros and Tsuang, Schizoaffective Psychoses, Springer-Verlag, 1986) supported the assumption that schizoaffective disorders differ in relevant ways from schizophrenic disorders. The classification of schizo affective disorders as a subgroup of schizophrenia has also been criticized, and empirical research in clinical, genetic, therapeutic, and prognostic areas supports the idea that there are some strong similari ties between schizo affective and affective disorders. Of course, there are not only similarities between these two groups, but also differences just as there are between schizo affective and schizophrenic disorders. It is precisely the existence of similarities and differences between schizo affective disorders and the other two so-called typical mental disorders, i.e., schizophrenia and affective disorders, which makes them a challenge in psychiatric research, a challenge to the traditional dichotomy in the classification of disorders which originated with Kraepelin. This challenge is certainly proving fruitful in psychiatric research. These "cases in between" may well demonstrate that sep arating, dividing, and limiting is not always meaningful. Sometimes it can be more meaningful to unify; to unify in the sense of building bridges between typical groups. It is here that the assumption of a "psychotic continuum" can become relevant, and the investigation of schizoaffective disorders is of prime importance in research on a possible continuum of psychosis.
The Natural History of Mania, Depression, and Schizophrenia takes an unusual look at the course of mental illness, based on data from the Iowa 500 Research Project. The brain is the most complex organ in our body. Indeed, it is perhaps the most complex structure we have ever encountered in nature. Both structurally and functionally, there are many peculiarities that differentiate the brain from all other organs. The brain is our connection to the world around us and by governing nervous system and higher function, any disturbance induces severe neurological and psychiatric disorders that can have a devastating effect on quality of life. Our understanding of the physiology and biochemistry of the brain has improved dramatically in the last two decades. In particular, the critical role of cations, including magnesium, has become evident, even if incompletely understood at a mechanistic level. The exact role and regulation of magnesium, in particular, remains elusive, largely because intracellular levels are so difficult to routinely quantify.
