This is a research-based book on the clinical use of the Herbst appliance in the management of Class 2 malocclusions. Different clinical problems and questions are addressed in the light of the corresponding research existing. Thus, in contrast to other Class 2 alternative treatments, the Herbst appliance approach is based on scientific research.

From an award-winning author, this concise and visual guide describes the two-phase treatment approach to the orthodontic management of children with uncrowded Class II division one malocclusion. In this widely accepted treatment method, a bionator (Andresen) appliance is used in phase one, followed in phase two by high quality preadjusted fixed appliances. Properly managed, and with correct case selection and timing, this system of treatment produces high-quality results, and can be the easiest route to a successful outcome in many patients. A new book by one of the most successful orthodontics authors in the world which offers a concise and easily assimilated account of the treatment of an important group of orthodontic patients. Describes the two phase treatment approach a functional appliance in phase one, and the latest and best in bracket and wire technology in phase two. This produces consistently high-quality results and is the easiest route to an ideal outcome in many patients. Stage by stage case histories enhance the readers understanding of the system. Lavishly illustrated with top-quality colour photographs and line diagrams. Has a focus on methods of optimising patient cooperation, a topic which is often overlooked. Discusses difficult situations where growth or cooperation levels are below expectations.

The book offers a comprehensive and critical review which presents not only the principles and techniques involved in the use of skeletal anchorage techniques and devices (such as orthodontic implants, miniscrew implants and mini plates), but also the scientific evidence available regarding the use of these contemporary applications and their clinical efficacy. • Provides an introduction to the conventional and noncompliance treatment of Class II malocclusion • Provides an introduction to the use of skeletal anchorage reinforcement approaches in orthodontics • Outlines the clinical considerations required for the use of skeletal anchorage devices in orthodontics • Explains the insertion and removal procedures of orthodontic implants, miniscrew implants and mini plates • Discusses the use of orthodontic implants for the treatment of Class II malocclusion • Explains the use of mini plates and zygomatic anchorage for the treatment of Class II malocclusion • Discusses the use of mini-screw implants for the treatment of Class II malocclusion • Explains the use of skeletal anchorage reinforcement of the noncompliance devices used for the treatment of Class II malocclusion • Explores the efficiency of skeletal anchorage and its risk management

The initial eight chapters provide a synthesis of the wide variety of treatments currently available for malocclusions in the sagittal, transverse, and vertical dimensions. The subsequent chapters describe the details of clinical management of fixed appliance therapy (e.g., bracket placement and design - including self-ligating brackets, orthodontic bonding, archwire sequencing, transpalatal arch and utility arch management). Dentofacial orthopedics, including rapid maxillary expansion and extraoral traction, as well as functional jaw orthopedics (Herbst, Twin Block, FR-2 of Fränkel, Bionator) and molar distalizing appliances (Pendulum, Distal Jet, Wilson distalizing arch) then are considered. The management of Class III malocclusion with the orthopedic facial mask, the chin cup and the FR-3 of Fränkel also are discussed in detail. The book also includes two extensive chapters on interdisciplinary treatment written by Vincent G. Kokich, as well as chapters on finishing and retention protocols. The clinical and cephalometric evaluation of the orthodontic patient and the relationship of orthodontic treatment, occlusion, and temporomandibular disorders also are presented. In essence, this book represents an extensive continuing education experience for the practicing orthodontist.

Anchorage control is one of the most challenging tasks in orthodontic treatment. Many different types of appliance are used to control anchorage, but an excellent outcome may be difficult to achieve owing to either poor mechanics or inadequate patient compliance. Recently, temporary skeletal anchorage devices (TSADs) have become popular in orthodontics. Some orthodontic movements that are now possible using TSADs were previously considered almost impossible with traditional orthodontic appliances. Several different types of TSAD are currently available, and in choosing between them orthodontists are obliged to rely on the information provided by manufacturers, which is often not based on scientific evidence. This book therefore presents the various design characteristics of TSADs and provides up-to-date scientific evidence to assist orthodontists in selecting the best TSADs for their patients.
