It has been a pleasure to comply with requests to publish this book in English. During the intervening years, there has been little to add to our views as to the best management of acetabular fractures, but an additional chapter has been incorporated comprising recent findings in our patients and slight changes in emphasis on the indications for operations. Additionally, having recognised that one of the greatest difficulties in this method of treatment lies in the pre-operative assessment of the standard radiographs, we have prepared a short series of radiographs which the reader may find advantageous for study. We are grateful to Mr. Reginald Eison who has translated and revised the French edition. Considerable alteration of the text and the general presen tation was necessary in order to make the material palatable in English. JUDET Preface to the French Edition It is a long time since we first attempted surgical treatment of fractures of the acetabulum accompanied by displacement, with the aim of restoring perfect articulation. Such treatment demands an exact reconstitution of the anatomy of the acetabulum and pelvic bone. This volume comprises an account of our efforts to assess the place of open reduction and internal fixation of displaced fractures of the acetabulum. The principal aim is simple: the perfect restoration of the articular surface and the associated bony architecture.

Although the number of published papers on traumatic hip disloca tion in childhood is large, our actual current knowledge on this inju ry in this age group is still full of obscure points and unsolved prob lems. Luxation of the hip of traumatic origin in children is infrequent, and the small number of cases reported by authors and institutions, as well as the usually short follow-up in many cases, are responsible for the many doubts and questions, especially related to complications and outcome following the injury. Consequently the treatment, especially the postreduction management, has not been adequately defined. Dr. Antonio Barquet has made the great effort to analyze the ex perience gained from 38 cases in Uruguay and to review more than 500 references related to the subject, covering more than 1400 cases. He began to study this topic in 1976. It was the subject of his postgraduate monograph in 1979, which was followed by a statisti cal analysis of the major complication, i. e., avascular necrosis of the proximal femoral extremity, which was presented as his doctoral thesis in 1981. In addition, he has published numerous papers deal ing with several aspects of traumatic hip dislocation in children.

For more than 100 years, congenital dislocation of the hip has been an area of concern in orthopedics. This publications on the subject are al most too numerous to count. Yet our knowledge of the basic principles of congenital hip dislocation and its management is constantly being expanded by new research. In Germanspeaking countries, Kaiser pub lished the last comprehensive textbook on congenital hip dislocation in 1958, and Schlegel followed with a comprehensive handbook in 1961. In the Angloamerican world, Coleman's monograph was pub lished in 1978, Somerville's in 1982, and Wilkinson's in 1985. In 1982 Tachdjian compiled a volume on congenital hip dislocation that con tained contributions from 44 authorities. We believe it is more prudent to take an individualized approach based on arthro graphic findings and the degree of severity of pathologic changes. Less emphasis is placed here on the management of patients by age group.
