When first published in 1985, this book was readily welcomed by both students and practitioners of physical medicine. It was the first full English-language introduction to the work of a world authority in the field; it remains unique, but its success has prompted some revision. Completely revised for the third edition, this book continues to offer a thought-provoking account of musculoskeletal disorders which will deepen the understanding of all therapists.
The contents of this book are based almost exclusively on purely anatomical researches. These were stimulated by questions posed in clinical practice. The results are directed to practicing surgeons. Their chronological sequence leads to a step by step development of theoretical bases and to a progressive rejection of old conceptions. Especially in the field of orthopaedic surgery, a responsible attitude is possible neither without solid anatomical knowledge, nor without an idea of functional relationships. W. Roux had already demonstrated this and he wanted his works of functional anatomy to be considered from this point of view. He above all preoccupied himself with a uniform theory of functional adaptation. Thus it is understandable that the theories ofRoux formed the basis from which to start. Our own researches seemed at first to corroborate the ideas of Roux, at least in part. This is still evident in the monograph concerning fractures of the femoral neck. Later it appeared that ST. KROMPECHER had made a step forwards in the matter of chondrogenesis when he abandoned the shear theory postulated by Roux and held that compression was the only effective stimulus for the forma tion of cartilage. The research concerning the healing of fractures relies partly on the theory of KROMPECHER which was new at that time. In this Atlas I want to share with my fellow clinicians the fascina tion I experienced while discovering the marvels of embryonic development. Why haven't these marvels excited me before? I believe that the use of schemata and drawings or photographs of animal embryos, commonly used in textbooks of embryology, simply did not appeal to me as a clinician. Only actual photo graphs of human embryos can establish the bond necessary for interaction. Just imagine the excitement when you find out how many struc tures you can recognize in a 5-week-old embryo, barely measuring 1 cm in length. But our fascination does not stop here. The pro gression of changes taking place during the next 3 weeks is so rapid that at the time when the embryo measures 3 cm, all structures familiar to us are not only easily recognizable, but also already in their anatomical position. How can we hide our amazement when we realize that such a state of perfection is present in an embryo a bit longer than the distal phalanx of our little finger? At 8 weeks the embryonic period ends and the fetal one starts. Although the shape and the relative size of bones, joints, muscles, nerves, and vessels will undergo changes, the basic elements are all in place. This implies that major malformations must develop during the embryonic period.

Physical rehabilitation for walking recovery after spinal cord injury is undergoing a paradigm shift. Therapy historically has focused on compensation for sensorimotor deficits after SCI using wheelchairs and bracing to achieve mobility. With locomotor training, the aim is to promote recovery via activation of the neuromuscular system below the level of the lesion. What basic scientists have shown us as the potential of the nervous system for plasticity, to learn, even after injury is being translated into a rehabilitation strategy by taking advantage of the intrinsic biology of the central nervous system. While spinal cord injury from basic and clinical perspectives was the gateway for developing locomotor training, its application has been extended to other populations with neurologic dysfunction resulting in loss of walking or walking disability.
