Les difficultés au niveau de la forme et/ou du contenu du langage peuvent entraîner, chez certains enfants, des difficultés au niveau de l’utilisation. Monfort, Juarez Sanchez et Monfort Juarez (2005) parlent alors de troubles pragmatiques secondaires et montrent que ces enfants peuvent bénéficier d’une prise en charge pour certaines de leurs difficultés dans ce domaine. La trisomie 21 entraînant des troubles du langage oral, les enfants porteurs de cette pathologie peuvent donc présenter des troubles pragmatiques secondaires. Selon De Weck (2005), ces difficultés concernent principalement la maîtrise des conversations. Nous nous sommes donc demandé si un entraînement sur la gestion des thèmes de conversation pouvait améliorer les habiletés conversationnelles dans le cadre de cette pathologie. Dans le cadre d’une étude de cas auprès d’une enfant atteinte de trisomie 21, après avoir réalisé un pré-test permettant d’évaluer ses habiletés conversationnelles, nous avons réalisé un travail rééducatif de quatorze séances, centré sur la gestion des thèmes de conversation. Pour cela, nous avons proposé des activités formelles et fonctionnelles axées sur l’organisation locale et globale des conversations. La comparaison des résultats obtenus lors d’un post-test à ceux du pré-test, ainsi que l’analyse qualitative des corpus réalisés à partir des séances de prise en charge ont permis de montrer l’efficacité partielle d’une telle intervention chez cette enfant. La capacité de l’enfant à gérer l’organisation locale des conversations n’a pas progressé suite à notre travail rééducatif. Toutefois, ses comportements en communication ainsi que sa capacité à initier une conversation et à en maintenir le thème en répondant à des questions se sont améliorés.

L'expansion des recherches cliniques dans le domaine pragmatique conduit de plus en plus d'orthophonistes à prendre en compte cette dimension dans leur pratique. Il s'avère alors nécessaire de disposer d'outils pertinents pour évaluer la compétence pragmatique. Dans ce cadre, notre attention s'est portée sur un outil linguistique, l'analyse conversationnelle, qui permet l'étude de cette compétence dans un contexte écologique d'échange conversationnel. Pour étudier l'intérêt de l'utilisation d'un tel outil dans le cadre d'une évaluation orthophonique, nous avons réalisé une analyse conversationnelle auprès d'un enfant présentant des troubles pragmatiques du langage, en lien avec une dysharmonie psychotique et des troubles autistiques. L'analyse nous a permis d'approfondir l'observation de ces troubles chez cet enfant, et nous a apporté des informations pertinentes pour sa prise en charge. A partir de ces résultats, nous avons pu réfléchir à l'utilisation d'un tel outil pour l'évaluation de troubles pragmatiques dans une pratique orthophonique.

In The Origins of Comics: From William Hogarth to Winsor McCay, Thierry Smolderen presents a cultural landscape whose narrative differs in many ways from those presented by other historians of the comic strip.
