In past years significant advances have been made in both the conceptualization and development of innovative program models providing integrated assessment and concommitant treatment for dually disordered patients. The purpose of this volume is to provide the reader with information about the most current ideas and clinical interventions available. Although many of the models presented here have been successful only in clinical trials, and efficacy is not yet confirmed by controlled research, the principles used in the development of these models hopefully will have value in assisting clinicians, managers, and planners who have an immediate need to provide services to this population. This is the 50th issue of the quarterly journal New Directions for Mental Health Services.

This practice-enhancing volume assembles the latest innovative thinking on working with clients who have both mental health diagnoses and substance use disorders. Diagnosis is a central focus of the coverage, untangling the often-knotty considerations surrounding dual diagnosis and the complex issues surrounding treatment even in frequently seen combinations (e.g., depression/alcohol abuse). The section on practice emphasizes meeting patients where they are and making use of their community, cultural, and spiritual contexts in crafting interventions. And the book’s ambitious chapters on professional development describe training programs with the potential to produce the next generation of responsive, knowledgeable, and flexible therapists. Among the topics covered: · Comprehensive assessment of substance abuse and addiction risk in adolescents. · The relationship between attachment and addiction. · Addiction in the community: the role of emergency services. · Substance use during and after major crisis and disaster: a practitioner’s guide. · Practice, advocacy, and outreach: perspectives on addiction services. · Teaching the importance of developing the therapeutic relationship. New Directions in Treatment, Education, and Outreach for Mental Health and Addiction equips health and clinical psychologists, social workers, and addiction counselors and educators with a well-rounded understanding of a growing population, and a wealth of perspectives on effective new interventions.

It may precipitate mental illness worsen mental illness alleviate mental illness have no effect on mental illness. The relationship between the patient’s different mental health disorders and substance abuse is not necessarily always the same. Patients who have both a psychiatric disorder and a substance abuse problem cause most clinicians to throw up their hands in despair. The clinical problems that these "dual diagnosis" patients present are enor mously complex. Diagnostically, how is one to tell if disorders of mood and thinking, for instance, are signs of a mental illness or consequences of substance abuse? How is one to obtain important historic information when the patient may be unable or unwilling to provide it and there are no readily available collateral sources of information? In any case, why bother? Treatments for dually diagnosed patients are ineffective; patients won't stay in treatment; recidivism occurs at a very high rate. To make matters even more difficult, traditional health care reim bursement mechanisms do not provide for the multimodality clinical programs and special services needed by the patient who is both mentally ill and a substance abuser. So the clinician needs an effective bureaucratic strategy as well as a treatment strategy. For the most part, clinicians have handled the problem by ignoring it.
