The larval stage of the parasite Taenia solium can encyst in the central nervous system causing neurocysticercosis, which is the main cause of acquired epilepsy in the countries in which the parasite is endemic. Endemic areas are those with the presence (or likely presence) of the full life cycle of Taenia solium. The parasite is most prevalent in poor and vulnerable communities in which pigs roam free, open defecation is practiced, basic sanitation is deficient, and health education is absent or limited. Several tools are available for the control of Taenia solium. Preventive chemotherapy for Taenia solium taeniasis, which is directed at the adult tapeworm, is one of them. Other tools focus on pig management, pig vaccination and treatment, sanitation and hygiene, and community education. Three potential drugs--niclosamide, praziquantel, and albendazole--have been considered for use for preventive chemotherapy in Taenia solium taeniasis control programs through mass drug administration or targeted chemotherapy. In this Guideline, we provide recommendations for preventive chemotherapy in Taenia solium-endemic areas using niclosamide, praziquantel, or albendazole, including at which dose and in which population groups. The development of this Guideline is based on the latest standard World Health Organization methods for guideline development, including the use of systematic search strategies, synthesis, quality assessment of the available evidence to support the recommendations, and participation of experts and stakeholders in the Guideline Development Group and External Review Group. The recommendations are intended for a wide audience, including policymakers and their expert advisers, and technical and program staff at governmental institutions and organizations involved in the planning, implementation, monitoring, and evaluation of preventive chemotherapy programs for the control of Taenia solium.

Neglected tropical diseases (NTDs) affect more than 1 billion people globally, causing serious, long-term health effects. Timely, effective interventions such as preventive chemotherapy, which involves the regular administration of safe, effective medicines to a defined population irrespective of its infection status, reduce morbidity and mortality. The new NTD road map advocates preventive chemotherapy as a core intervention to reduce the burden of NTDs. Effective administration of preventive chemotherapy relies largely on operational planning which addresses the need for programmes to reach all eligible people and maximize community demand for the intervention. Local, bottom-up planning (“microplanning”) fosters effective implementation and monitoring of public health programmes. Originally, public health implementers developed microplanning for use in vaccination campaigns, but various programmes, including those for control and elimination of NTDs, now use it. This manual is based on these and other country experiences in microplanning of preventive chemotherapy for NTDs. It describes the basic concepts of microplanning and provides steps and examples of tools to develop and implement a microplan. Since microplanning originates at the lowest level of implementation, and is a bottom-up process, the main target audience of this manual is individuals who are responsible for oversight of preventive chemotherapy against NTDs.

"The presence, or absence, of neglected tropical diseases (NTDs) can be seen as a proxy for poverty and for the success of interventions aimed at reducing poverty. Today, coverage of the public-health interventions recommended by the World Health Organization (WHO) against NTDs may be interpreted as a proxy for universal health coverage and shared prosperity - in short, a proxy for coverage against neglect. As the world's focus shifts from development to sustainable development, from poverty eradication to shared prosperity, and from disease-specific goals to universal health coverage, control of NTDs will assume an important role towards the target of achieving universal health coverage, including individual financial risk protection. Success in overcoming NTDs is a "litmus test" for universal health coverage against NTDs in endemic countries. The first WHO report on NTDs (2010) set the scene by presenting the evidence for how these interventions had produced results. The second report (2013) assessed the progress made in deploying them and detailed the obstacles to their implementation. This third report analyses for the first time the investments needed to achieve the scale up of implementation required to achieve the targets of the WHO Roadmap on NTDs and universal coverage against NTDs. INVESTING TO OVERCOME THE GLOBAL IMPACT OF NEGLECTED TROPICAL DISEASES presents an investment strategy for NTDs and analyses the specific investment case for prevention, control, elimination and eradication of 12 of the 17 NTDs. Such an analysis is justified following the adoption by the Sixty-sixth World Health Assembly in 2013 of resolution WHA6612 on neglected tropical diseases, which called for sufficient and predictable funding to achieve the Roadmap's targets and sustain control efforts. The report cautions, however, that it is wise investment and not investment alone that will yield success. The report registers progress and challenges and signals those that lie ahead. Climate change is expected to increase the spread of several vector-borne NTDs, notably dengue, transmission of which is directly influenced by temperature, rainfall, relative humidity and climate variability primarily through their effects on the vector. Investments in vector-borne diseases will avoid the potentially catastrophic expenditures associated with their control. The presence of NTDs will thereby signal an early warning system for climate-sensitive diseases. This document is the second in a series of global reports describing progress towards the 2030 targets set in Ending the neglect to attain the Sustainable Development Goals: a road map for neglected tropical diseases 2021–2030. It describes a wide range of activities, accomplishments and challenges across the portfolio of NTDs and across all six WHO regions. The report presents epidemiological and programmatic data for 2022, which were gathered, compiled and analysed in 2023. In some cases, 2023 data are available and presented; in other cases, less recent information is included, when 2022 data are not available. In addition, it presents the main facts or events that occurred in 2023. In line with the road map’s companion document Ending the neglect to attain the Sustainable Development Goals: a framework for monitoring and evaluating progress of the road map for neglected tropical diseases 2021−2030, the report includes quantitative information on the status of the overarching, cross-cutting and disease-specific indicators. This is followed by qualitative information on each of the three road map pillars and on regional and country progress.
