During the past 30 years, the Study Group for the Problems of Osteosyn thesis (AO) has made decisive contributions to the development of osteo synthesis as a surgical method. Through close cooperation among special ists in the fields of orthopedic and general surgery, basis research, metallurgy, and technical engineering, with consistently thorough follow up, it was possible to establish a solid scientific background for osteosyn thesis and to standardize this operative method, not only for the more ob vious applications in fracture treatment, but also in selective orthopedics where hardly any problems relating to bone, such as those with osteoto mies can be solved without surgical stabilization. The Manual of INTERNAL FIXATION is well known internationally as a standard work for every specialist dealing with osteosynthesis. Immediately after the fracture and during the phase of repair, we see certain local circulatory disturbances, certain manifestations of local inflammation, as well as pain and reflex splinting. These three factors, that is, circulatory disturbances, inflammation and pain, when combined with the defunctioning of bone, joints and muscle, result in the so-called jl'acture disease.
The purpose of this book is to report on my 17 years of experience in the development and implementation of a closed functional method of treatment of certain fractures of long bones of the appendicular skeleton. My personal experiences, clinical results, and the basic concepts underlying the treatment philosophy are reported. Illustrations demonstrating the techniques of application of functional casts and braces are presented in detail as well as a step-by-step description of the management regime and a discussion of the behavior of fractures under this system. Indications and contraindications for the use of this method of treatment are clearly outlined. I consider it important to compile this information for I believe that the system has withstood the test of time and is sufficiently sound to be incorporated in the armamentarium of the orthopaedic surgeon. Also, it may have a favorable socioeconomic impact since it offers a viable alternative in the treatment of frac tures.
