Disasters and Public Health: Planning and Response, Second Edition, examines the critical intersection between emergency management and public health. It provides a succinct overview of the actions that may be taken before, during, and after a major public health emergency or disaster to reduce morbidity and mortality. When communities face complex public health emergencies, state local, tribal, and territorial public health agencies must make difficult decisions regarding how to effectively respond. The public health emergency preparedness and response (PHEPR) system, with its multifaceted mission to prevent, protect against, quickly respond to, and recover from public health emergencies, is inherently complex and encompasses policies, organizations, and programs. Since the events of September 11, 2001, the United States has invested billions of dollars and immeasurable amounts of human capital to develop and enhance public health emergency preparedness and infrastructure to respond to a wide range of public health threats, including infectious diseases, natural disasters, and chemical, biological, radiological, and nuclear events. Despite the investments in research and the growing body of empirical literature on a range of preparedness and response capabilities and functions, there has been no national-level, comprehensive review and grading of evidence for public health emergency preparedness and response practices comparable to those utilized in medicine and other public health fields. Evidence-Based Practice for Public Health Emergency Preparedness and Response reviews the state of the evidence on PHEPR practices and the improvements necessary to move the field forward and to strengthen the PHEPR system. This publication evaluates PHEPR evidence to understand the balance of benefits and harms of PHEPR practices, with a focus on four main areas of PHEPR: engagement with and training of community-based partners to improve the outcomes of at-risk populations after public health emergencies; activation of a public health emergency operations center; communication of public health alerts and guidance to technical audiences during a public health emergency; and implementation of quarantine to reduce the spread of contagious illness.
Chemical, Biological, Radiological, Nuclear, and high-yield Explosive (CBRNE) events have the potential to destabilize governments, create conditions that exacerbate violence or promote terrorism. This can trigger global repercussions. These events can quickly overwhelm the infrastructure and capability of the responders, especially in countries that do not have the specialized resources for response like those available in the United States. When a CBRNE incident occurs in a partner nation or other foreign country, the U.S. is often called upon to provide assistance. Interoperability - the ability to work together - among U.S. agencies, foreign governments, and responders involved in the effort is key to an efficient response. The effectiveness of the U.S. response and approach to CBRNE events in partner nations depends on the capability of the U.S. government to provide timely and appropriate assistance and the resilience of the partner nation to a CBRNE event. An All-of-Government Approach to Increase Resilience for International Chemical, Biological, Radiological, Nuclear, and Explosive (CBRNE) Events is the summary of a workshop convened in June 2013 by the National Institute of Standards and Technology and the National Research Council to discuss ways to strengthen the U.S. ability to prepare for and respond to CBRNE events that occur in U.S. partner nations. The workshop brought together diverse experts and stakeholders to identify capabilities that are necessary for responding to an international CBRNE event; discuss best practices and resources needed for improved interoperability of the U.S. and partner nation during response to a CBRNE event; and identify key questions that need to be addressed in follow up activities focused on improving U.S. CBRNE response in partner nations.
Disaster management is an increasingly important subject, as effective management of both natural and manmade disasters is essential to save lives and minimize casualties. This book discusses the best practice for vital elements of disaster medicine in both developed and developing countries, including planning and preparedness of hospitals, emergency medical services, communication and IT tools for medical disaster response and psychosocial issues. It also covers the use of state-of the-art training tools, with a full section on post-disaster relief, rehabilitation and recovery.
During a wide-reaching catastrophic public health emergency or disaster, existing surge capacity plans may not be sufficient to enable health care providers to continue to adhere to normal treatment procedures and follow usual standards of care. This is a particular concern for emergencies that may severely strain resources across a large geographic area, such as a pandemic influenza or the detonation of a nuclear device. Under these circumstances, it may be impossible to provide care according to the standards of care used in non-disaster situations, and, under the most extreme circumstances, it may not even be possible to provide basic life sustaining interventions to all patients who need them. Although recent efforts to address these concerns have accomplished a tremendous amount in just a few years, a great deal remains to be done in even the most advanced plan. This workshop summary highlights the extensive work that is already occurring across the nation. Specifically, the book draws attention to existing federal, state, and local policies and protocols for crisis standards of care; discusses current barriers to increased provider and community engagement; relays examples of existing interstate collaborations; and presents workshop participants' ideas, comments, concerns, and potential solutions to some of the most difficult challenges.
Today our emergency care system faces an epidemic of crowded emergency departments, patients boarding in hallways waiting to be admitted, and daily ambulance diversions. Comprehensive Preparedness Guide (CPG) 101 provides guidelines on developing emergency operations plans (EOP). It promotes a common understanding of the fundamentals of risk-informed planning and decision making to help planners examine a hazard or threat and produce integrated, coordinated, and synchronized plans. The goal of CPG 101 is to make the planning process routine across all phases of emergency management and for all homeland security mission areas. This Guide helps planners at all levels of government in their efforts to develop and maintain viable all-hazards, all-threats EOPs. Accomplished properly, planning provides a methodical way to engage the whole community in thinking through the life cycle of a potential crisis, determining required capabilities, and establishing a framework for roles and responsibilities. It shapes how a community envisions and shares a desired outcome, selects effective ways to achieve it, and communicates expected results. Each jurisdiction's plans must reflect what that community will do to address its specific risks with the unique resources it has or can obtain.
