Background: in 1970's the seminal work of Cohen and Jago initiated an interest in the measurement of health related quality of life measures in dentistry. Clinical measures to assess oral health from the dentist's perspective do not necessarily measure well-being. The patient reported outcome measures are an important tool to measure and improve the quality of care. -- Aim: The aim of the project was to create and validate a brief instrument for measuring patient-based oral health outcomes associated with endodontic care. -- Method: Based on the literature review of the currently used core oral health outcome measures, items from established instruments were selected to generate pool of items for the new measure. All subjects attending primary dental care at a local dental teaching hospital specifically for endodontic treatment provided by undergraduates self-completed this outcome measure prior to, immediately following care and at one month follow-up visit. Using multicollinearity, factor analysis, regression modelling and an expert based approach items were identified for a brief instrument. The newly developed brief instrument was tested for its reliability, validity and responsiveness on patients attending for endodontic treatment in the same clinical setting using the self-completed questionnaire at the baseline and after completion of the treatment. -- Results: 46 Items from OHIP-49 (Oral Health Impact Profile) and 12 items from GOHAI (General Oral Health Assessment Index) were used as the basis for development of an endodontic outcome measure. Multicollinearity, factor analysis and regression analysis of the long-form of the instrument (58 items) identified 15 significant items associated with improved oral health.

Background. Evidence based Dentistry and Patient Centered Care are the two paradigms which influence the process of clinical decision making in modern dental care. Although they are complementary means to improve quality, they may seem at odds with each other. In the last decade, a need has emerged to examine the intersection of these two paradigms. Meta-analyses are a hallmark of evidence based dentistry as they succeed in showing statistically significant results by combining the results from individual studies. However, they are limited when the individual studies are heterogeneous. Meta-regression may help investigate this heterogeneity, but its limited ability to identify which patient features are related to the size of treatment effect is answered by using an individual patient data approach. Poor reporting of individual patient data meta-analysis diminishes its value to clinicians, policy makers, and other users. Our goal is to develop and validate a tool to assess the quality of an individual patient data meta-analysis. Methods. We develop a tool based on the literature available in the field of Individual patient data meta-analysis. A modification of the PRISMA is done without altering its intent of use. In total, twelve Individual patient data meta-analyses in oral and maxillofacial medicine are identified to validate the tool and to establish its reliability. This is carried out by two independent readers who are standardized and trained to avoid misinterpretation of the Tool items. A Pearson's r coefficient is calculated to test inter-rater; intra-rater reliability and criterion validity. Results A 9 item tool with a minimum of 3 criteria per item is constructed. The lowest score possible per question is 1 and the highest score possible per question is 4. The minimum total score possible is 9 and the maximum total score possible is 36. The new instrument has high content validity by virtue of its construction process. Testing the tool for criterion validity yields a Pearson's r of 0.957. The tool has construct validity based on the evidence of Test content, Internal structure, Response process, Relations to other variables, and Consequences of testing. It is also reliable with a Pearson's r of 0.927 for inter-rater reliability and a Pearson's r of 0.972 and 0.904 for intra-rater reliability. Discussion Our study led to the development of a novel instrument specifically designed for assessing the quality of individual patient data meta-analysis. The tool scores for the 12 papers quantified the degree to which each paper satisfied certain criteria that are established in the literature as determinant factors of highest quality meta-analysis in general, and individual patient data meta-analysis in particular. The tool has significant implications for clinicians and researchers. We recognize the need for further testing of the Tool to increase its validity. New evidence in areas of methodology of IPDMA may call for a need to update the Tool.

Patient-based research provides insight into real-world endodontics. Cross-sectional studies describe the health status of populations. Root canal-associated pain is a source of fear for patients, a concern of dentists, and is remembered and shared. Inclusion/exclusion criteria and defined searches selected articles in MEDLINE, EMBASE, Cochrane, and PsycINFO databases. Prevalence of periapical radiolucency and root canal treatment were calculated. L'Abbe plots evaluated the influence of root canal treatment on pain prevalence and severity. 5% of teeth had periapical radiolucencies, 9% endodontically treated, 37% of treated teeth had periapical radiolucencies, and 2% of untreated teeth had periapical radiolucencies. Pre-treatment, 24 hour, and 1 week pain prevalences were 81(28)%, 40(24)%, and 11(14)%. Pre-treatment, 24 hour, and 1 week pain severities, were 54(24)%, 24(12)%, and 5(5)%. L'Abbe plots revealed a decrease in pain prevalence and severity. Prevalence of periapical radiolucency and root canal treatment was very high. Pain prevalence and severities decreased after treatment.

Access to oral health care is essential to promoting and maintaining overall health and well-being, yet only half of the population visits a dentist each year. Poor and minority children are less likely to have access to oral health care than are their nonpoor and nonminority peers. Older adults, people who live in rural areas, and disabled individuals, uniformly confront access barriers, regardless of their financial resources. The consequences of these disparities in access to oral health care can lead to a number of conditions including malnutrition, childhood speech problems, infections, diabetes, heart disease, and premature births. Improving Access to Oral Health Care for Vulnerable and Underserved Populations examines the scope and consequences of inadequate access to oral health services in the United States and recommends ways to combat the economic, structural, geographic, and cultural factors that prevent access to regular, quality care. The report suggests changing funding and reimbursement for dental care; expanding the oral health work force by training doctors, nurses, and other nondental professionals to recognize risk for oral diseases; and revamping regulatory, educational, and administrative practices. It also recommends changes to incorporate oral health care into overall health care.
